Welcome to HealthOutreach® A FREE program that promotes health and quality of life for adults 60 years of age or older. Sponsored by NewYork-Presbyterian Hospital, HealthOutreach is a unique membership program offered at NO COST to you. Our goal is to enhance the social, physical, emotional and educational well being of older adults through a variety of programs. As a member, you can select from a complete list of services ranging from health lectures, workshops, and classes to individualized counseling, support groups, and social events, as well as assistance for caregivers. What’s more, HealthOutreach offers easy access to hospital and community services, and can provide you with ongoing help with your insurance claims and benefits issues, prescriptions, physician referrals, and other vital information. Our focus is on promoting healthy, active living through a wide range of programs, information, and services—created just for you.
