About Us

NewYork-Presbyterian Hospital administration and the Department of Social Work recognize the importance of providing improved access to hospital services and healthcare education resources to the older adult community. HealthOutreach was created in the late 1980’s to address this need by offering ongoing community support for older adults and guidance for caregivers—at no charge. Now for more than a decade, HealthOutreach has successfully served nearly 10,000 adults aged 60 years and older throughout the New York metropolitan area.
