	About Us | Meet our staff HeathOutreach is staffed by experienced New York board certified social workers who are available to help you navigate the healthcare system and provide support for your physical, social, and emotional needs. Each center is lead by a Program Director dedicated to your personal needs. Our goals are to improve the quality of life for older adults, families, and caregivers and to supply education and training about managing your health, obtaining medical care, and related issues that impact our senior community today. The HealthOutreach Program Directors: Christopher Oates, CSW, is the Program Director for HealthOutreach at NewYork- Presbyterian Hospital, NewYork Weill Cornell Medical Center. He is a graduate of Columbia University School of Social Work with training in geriatrics and HIV/AIDS. Under his direction, the HealthOutreach program has grown to serve nearly 10,000 older adults and caregivers throughout the New York metropolitan area. Thomas Sedgwick, CSW ACSW, is the Director of HealthOutreach at NewYork-Presbyterian Hospital, The Allen Pavilion. Mr. Sedgwick established the HealthOutreach Program at The Allen Pavilion in 1998. He is a graduate of The Columbia University School of Social Work and has special training in the areas of geriatrics and medical/surgical social work. In addition to directing HealthOutreach, Mr. Sedgwick manages the social work department at The Allen Pavilion. Our directors and other specially trained staff members are also available to listen to your questions, evaluate your needs, and assist you with referrals to appropriate hospital and community resources.
