But now you can obtain the quality of care and life you need through the Geriatric Care Management Program. The Geriatric Care Management Program is a fee-for-service program available through HealthOutreach that helps older adults and their caregivers obtain the advice, support and care they need. We can aid you in all aspects of short and long-term geriatric care. Whether it’s refining and enacting a plan you’ve developed or helping you to take the first steps toward creating a more secure, comfortable life—we’re available to assist you.
